  UPFRONT BRIEFING

AstraZeneca went looking for $1.5B a year down the back of the sofa: a three-year savings programme, roughly $2B in one-off charges, and every dollar recycled into R&D before Farxiga's generic slide gets steeper. Regeneron needed no cushion-diving. Q3 revenue rose 36% to $5.1B on a $6.5B Dupixent quarter, and Leonard Schleifer says he's open to deals at every stage.

Less fun at Acumen: its oligomer-selective Alzheimer's antibody missed in a 542-patient Phase 2 and the company is scrapping its only clinical asset. This week: SITC opens Wednesday in Phoenix and AASLD Thursday in Denver, where Regeneron brings interim CIDEB liver-fat data, while Agios now waits until 1 February for its sickle cell call.

TAPE ACTION

  • Healthcare held green on a heavy down day: XLV +0.5% vs S&P (1.8%), NBI flat, XBI (1.1%), BBC (1.9%). Regeneron rose about 12% after its pre-open Q3 beat, while Acumen fell roughly 80% after scrapping sabirnetug.

  • Intel fell 28.6%, its worst day since 1974, and Amazon's results added to worries over AI spending and cash burn: chip index (7.2%), AMD (10.9%), Nvidia (4.2%). NDX (3.5%), S&P (1.8%), Dow (1.1%), RUT (1.7%); the Nasdaq Composite ended October 7.8% below its 6 Oct closing high.

THE BIG 3

1️⃣ — AstraZeneca launches $1.5B savings and reorg plan

  • AstraZeneca launched a three-year programme targeting $1.5B in annual cost savings by end-2029, with about $2B in one-off charges and the savings recycled into R&D. Management says net headcount won't shrink but flagged that the reorganisation could mean R&D cuts and more trial activity shifted to emerging markets. It landed alongside Q3 results and follows GSK's own $1.5B savings push.

  • Why it matters: This is a funding move dressed as a cost programme. AstraZeneca is still guiding to $80B of revenue in 2030 while Farxiga fell 25% to $1.65B in Q3 on generics, and Lynparza, Imfinzi and Tagrisso lose exclusivity by the mid-2030s. Pascal Soriot sizes the 2027 readout slate at more than $20B of potential, and $1.5B a year is what it costs to run those trials without squeezing margin. The no-net-job-loss pledge keeps the politics quiet; the emerging-market trial shift is the operational tell, trading cheaper sites and faster enrolment for more FDA scrutiny of ex-US data. GSK announced its own $1.5B savings push this month, so the large-cap template is now set.

2️⃣ — Regeneron's revenue jumps 36% as Dupixent clears $6.5B

  • Regeneron's Q3 revenue rose 36% to $5.12B against roughly $4.56B expected, with non-GAAP EPS of $20.07 vs about $16.2 consensus. Dupixent global sales (booked by Sanofi) rose 35% to $6.5B, US Eylea HD rose 62% to $699M while legacy Eylea fell 41% to $400M, and Libtayo grew 51% to $550M. Shares rose about 12% on Friday.

  • Why it matters: The beat is mostly Sanofi. Collaboration revenue rose 68% to $2.72B, roughly four-fifths of the year-on-year gain, so the quality question is Dupixent's runway to its 2031 US patent expiry rather than the eye franchise. Eylea HD is now absorbing the original Eylea's erosion to Amgen's Pavblu, leaving combined US Eylea sales down only about 1%, which takes the sting out of the 2025 bear case. The next tests come quickly: an FDA decision on cemdisiran in generalised myasthenia gravis this month, interim CIDEB RNAi liver-fat data at AASLD, and odronextamab's February 2027 DLBCL date. Leonard Schleifer told analysts Regeneron is open to deals at every stage, with $2.2B left on the buyback.

3️⃣ — Acumen scraps sabirnetug after 542-patient Alzheimer's Phase 2 miss

  • Acumen's ALTITUDE-AD Phase 2 (n=542, early Alzheimer's, monthly infusions at 35 or 50 mg/kg) missed its primary endpoint, iADRS change at 18 months, at both doses, and CDR-SB showed no meaningful separation from placebo. Plaque fell significantly on PET at weeks 36 and 76, and ARIA-E ran at about 10%. Acumen is discontinuing sabirnetug, including the open-label extension, and its shares fell about 80%.

  • Why it matters: This was the cleanest test yet of the soluble-oligomer hypothesis, and it failed on function despite moving plaque. Sabirnetug was built to hit the toxic amyloid-beta oligomers that plaque-clearing antibodies reach only indirectly, with the pitch of Leqembi-type benefit at lower ARIA risk. The safety half held at roughly 10% ARIA-E, but plaque reduction with no cognitive separation at 18 months undercuts the idea that oligomer selectivity was the missing ingredient. The read for Lilly's Kisunla and Eisai's Leqembi is mildly supportive: high-dose plaque clearance remains the only validated route. Acumen keeps its JCR-partnered Enhanced Brain Delivery programme and is exploring strategic options, so a reverse-merger shell is the likely end state.
