UPFRONT BRIEFING
Hepatitis B finally has a drug aiming at a cure rather than lifelong suppression: the FDA cleared GSK and Ionis' Hibsago late Tuesday, and roughly one in five patients clears the virus, with Ionis pocketing 10–12% royalties on every dose GSK sells. Roivant stacked two Phase 3 uveitis wins onto Lisraya, and Alcon wrote a $550M check for Neurotech's approved MacTel implant, proof that de-risked eye assets still find buyers.
The swing factor sits in Washington, where Treasury's draft outbound-investment rules, expected today, could make China licensing a longer trip. Rezera, the former NodThera, filed to list. MSToronto wraps today with Roche talking up fenebrutinib, ASN Kidney Week runs through Sunday, and ESMO opens in Madrid tomorrow with Merck/Moderna and Summit/Akeso among the headliners.
TAPE ACTION

Healthcare gave back part of Tuesday's record, with XLV (0.5%) among the S&P's weaker sectors, XBI (0.3%) and BBC (0.5%) soft, and cap-weighted NBI up 0.2%. Roivant traded up on CLARITY data (ROIV ~+10% premarket).
Tech carried the tape, with the sector up 1.7%, the Nasdaq Composite +1.1% and the Nasdaq 100 +1.1% against the S&P's +0.6%, while communication services (1.1%) lagged; the Fed is in blackout before its 27–28 Oct meeting, and Meta, IBM and Lam Research reported after the bell.
THE BIG 3
1️⃣ — GSK, Ionis land first FDA nod for a hepatitis B functional cure
The FDA approved Hibsago (bepirovirsen) late Tuesday under priority review for adults with chronic hepatitis B without cirrhosis, added to NRTI therapy after at least six months on it with HBsAg at or below 3,000 IU/mL; a six-month course, with self-injection after the first dose.
Why it matters: B-Well 1 and 2, pooled, delivered functional cure (HBsAg loss plus undetectable HBV DNA 24 weeks off treatment) in 19% of patients (233/1,220) versus none on placebo, rising to 26% with baseline HBsAg at or below 1,000 IU/mL; NRTIs alone manage under 1% a year. Jefferies pencils in peak sales of £2.2B (~$2.95B), and Ionis collects 10–12% tiered royalties plus sales milestones. However, a boxed warning for hepatic flares and low platelets, plus weekly-to-fortnightly labs that Jefferies expects to keep early use inside hepatology centres. GSK is concentrating its bets, ending the Phase 1 PAPD5/7 inhibitor GSK3965193 and leaving siRNA GSK5637608 in Phase 2 as its main follow-on.

2️⃣ — Roivant's brepocitinib halves uveitis treatment failure in twin Phase 3s
Brepocitinib 45mg met the primary endpoint in both CLARITY-1 (n=166) and CLARITY-2 (n=204) in non-infectious uveitis: treatment failure by week 48 was 35.4% vs 78.7% (HR 0.27) and 32.4% vs 77.8% (HR 0.22), both p<0.0001; supplemental filing in 1H27.
Why it matters: Every key secondary hit: pooled steroid-free quiescence of 62% versus 20%, and macular edema resolution of 42% versus 4.8%, with only 3% discontinuing for adverse events. The release gives no SAE, infection or VTE breakdown, which is where JAK-class labels get written. The incumbent is AbbVie's Humira; there is no head-to-head, but Stifel calls brepocitinib potentially best-in-disease and Cantor sizes non-infectious uveitis at $5B peak across 70,000–190,000 US adults. A second indication turns Lisraya, approved in dermatomyositis in August, into a TYK2/JAK1 franchise rather than an orphan launch. ROIV traded up ~10% premarket, and Pfizer still owns roughly a quarter of Priovant.

3️⃣ — Alcon pays $550M upfront for Neurotech's MacTel implant Encelto
Alcon agreed to acquire privately held Neurotech for $550M upfront plus undisclosed commercial milestones, taking Encelto, the only FDA-approved therapy for macular telangiectasia type 2 (cleared March 2025), and its encapsulated cell therapy platform.
Why it matters: The price sits between Alcon's $355M Simbrinza rights deal and its $770M Aerie buyout. Encelto is a surgically placed implant whose engineered cells release ciliary neurotrophic factor continuously; Neurotech counts ~25,000 US MacTel patients with 2,000–3,000 new diagnoses a year, and sales are undisclosed. Alcon guides to modest 2027 EPS dilution before accretion, so the price buys the encapsulated cell therapy platform as much as one orphan label: a sustained-release alternative to the repeat intravitreal injections that set retina economics. The read-across is to sustained-delivery developers still burning cash, where EyePoint's Duravyu just missed non-inferiority to Eylea and the company halved headcount.
