Upfront Briefing

Novartis wrote a $125M cheque for Sironax's brain-delivery platform on the same day its TREM2 bet in ALS came up empty. Buying the delivery layer while a biology bet fails is about as on-message as a rough month gets.

Vera brought two years of kidney-function receipts for Trutakna, and Skyhawk says its oral Huntington's drug is holding patients steady at 15 months, with an external control doing a lot of the lifting.

  • Biotech held flat-to-green through the Fed decision, with XBI up 0.1%, NBI up 0.2% and XLV up 0.1% against a (0.4%) S&P 500. No single sector catalyst stood out; the steadier session followed Tuesday’s 2.3% XBI slide as the 10-year sat at 5%.
  • The Fed raised rates 25bp to 3.75%–4.00%, its first hike since 2023, and 16 of 18 officials projected at least one more this year. Stocks gave up early gains during Chair Warsh’s press conference as he stressed that inflation remains too high; the Dow fell 1.2%.

Novartis buys a brain shuttle as its TREM2 bet in ALS fails
  • Novartis exercised its option to buy Sironax's Brain Delivery Module platform for $125M at closing, inside the up to $175M in upfront and near-term payments set in a July 2025 option and asset purchase agreement; Sironax keeps rights to selected assets. Separately, Novartis dropped VHB937 (lifonebart), a TREM2 stabiliser, in ALS after the 251-patient Phase 2 ASTRALS trial missed its primary and secondary endpoints.
  • Why it matters: The $125M is a closing payment on a platform Novartis spent 14 months testing, which makes it a conviction buy rather than an option premium. Antibody delivery is the bottleneck in neurodegeneration, and Novartis now owns a modality-agnostic shuttle alongside its $200M-upfront Arrowhead ARO-SNCA licence; Roche paid Manifold Bio $55M upfront for shuttle work last November. The ALS miss extends a month that already included the del-desiran HARBOR failure, the pelacarsen stumble and paused autoimmune CAR-T trials. TREM2 activation has now failed in the clinic for Alector, Vigil and Novartis, so the ongoing lifonebart Alzheimer's Phase 2 carries the mechanism. Sironax keeps its SARM1 inhibitor SIR2501 in ALS, funded by the closing cash.

Vera's Trutakna holds kidney function flat over two years
  • Final ORIGIN 3 analysis in 428 adults with IgA nephropathy: eGFR moved −0.1 vs −5.7 mL/min/1.73m² on placebo at 52 weeks (difference 5.6, p<0.0001), the two-year annualised slope was −0.6 vs −5.6, and composite progression events fell 76% (11 vs 38). Vera plans a full-approval sBLA in Q4; data disclosed Tuesday 15 September.
  • Why it matters: Trutakna launched in July on proteinuria alone, and this is the eGFR evidence payers wanted before backing a $425,000 list price, above roughly $390,000 for Otsuka's Voyxact. Voyxact's two-year VISIONARY slope was +0.3 vs −4.2 on placebo, so the two now look close on kidney function; different baseline risk makes any cross-trial ranking fragile. The bigger commercial threat is Vertex's povetacicept, a second dual BAFF/APRIL inhibitor with a 30 November PDUFA date, with Novartis' Fabhalta and Vanrafia also in the market. VERA rose 4% to $35.39 on Tuesday and is still down 34% for the year, which says investors are pricing a crowded class more than the data. More than 350 patient start forms in ten weeks is the launch number to watch.

Skyhawk's oral Huntington's drug holds function at 15 months
  • Final Phase 1/2 data for SKY-0515, a once-daily oral RNA-splicing modifier: cUHDRS improved 0.94 points from baseline at month 15 vs a 0.65-point decline in an overlap-weighted natural-history control, a 1.59-point difference (95% CI 1.09–2.09). The 9 mg dose cut mutant huntingtin protein by more than 60% and PMS1 mRNA by more than 25%.
  • Why it matters: The comparator is external, drawn from Enroll-HD and other natural-history cohorts, and the placebo-controlled window lasted only 12 weeks, so the functional signal stays hypothesis-generating until the pivotal Phase 2/3 FALCON-HD programme reads out. Mechanism is the differentiator: SKY-0515 lowers huntingtin and PMS1, a DNA-repair factor that drives somatic CAG expansion, in a pill. That sets it directly against votoplam, the PTC oral splicing modifier Novartis licensed for $1B upfront in December 2024, and ahead of intrathecal approaches on convenience. Skyhawk is private, so listed read-through runs to PTC and to a Novartis neuroscience unit short on wins. No serious adverse events so far is the other variable to track as the Phase 2/3 programme, now past 200 patients across both studies, scales.
