Novo spent a day in London explaining life after semaglutide, pinned a $23B 2035 pipeline-sales ambition on it, and got its worst Copenhagen session since February for the trouble. Grail had the opposite kind of Monday: FDA staff papers left very little to argue about ahead of Wednesday's Galleri panel.

  • Biotech trailed the broad rally: XBI rose 1.0% and NBI 1.1% against the S&P 500's 1.5%, while XLV added 0.4%. Single-name moves ran both ways, with Grail up 34% on FDA briefing papers posted before the open and Novo's ADR down about 8% after its Capital Markets Day.

Novo sets a $23B 2035 goal and loses 7.7% for it
  • At its London Capital Markets Day, Novo targeted more than DKK150B (~$23B) of risk-adjusted pipeline sales in 2035 and more than five multi-blockbuster launches by 2030; Copenhagen shares closed down 7.7%.
  • Why it matters: Novo put a number on life after semaglutide: more than DKK150B (~$23B) of risk-adjusted pipeline sales in 2035, current assets included, plus 2026–2030 revenue growth pitched only to match a peer set that includes Lilly, Merck and AstraZeneca. Matching peers is the concession. Management called semaglutide's loss of exclusivity early next decade the elephant in the room, and nothing in the plan bridges it before then. Copenhagen closed down 7.7%, the worst day since February, and the ADR was still off about 8% late in the US session. The Kallyope and Orbis deals of the past week look like early-stage breadth, not a replacement franchise, so the call for a larger acquisition gets louder. Next hard catalyst: the US CagriSema obesity decision, expected around year-end.

Grail jumps 34% as FDA staff clear the way to Galleri's panel
  • FDA briefing documents for Wednesday's (23 Sep) advisory panel on Galleri, Grail's multi-cancer blood test, raised no outstanding questions on analytical performance, study design or primary safety; GRAL rose 34%.
  • Why it matters: FDA staff left the panel no outstanding questions on Galleri's analytical performance, study design or primary safety analyses, and GRAL closed up 34% at about $108 from $80.77, its best day since February 2025. The one open question is narrower but real: reviewers asked whether the evidence supports calling Galleri an early detection test, which runs straight back to February, when NHS-Galleri failed to significantly cut late-stage diagnoses. The PMA leans instead on PATHFINDER 2's 25,490 participants and the first NHS-Galleri screening round of more than 70,000. A favourable vote matters mainly for Medicare coverage, which approval is expected to unlock. After a one-day re-rating, a split vote on labelling is now the downside case. Grail expects the FDA decision in the first part of 2027.

Pfizer's MFN contract hands HHS a cut of higher foreign prices
  • Pfizer and Lilly MFN contracts released by Public Citizen under FOIA show Pfizer sharing a portion of net increased revenue from higher ex-US prices with HHS; rates and covered products are redacted.
  • Why it matters: The contract Public Citizen obtained through a FOIA suit shows Pfizer ceding to HHS a share of the net incremental revenue it earns from raising prices abroad, on marketed drugs, from 1 January 2026 to 20 January 2029. The rate and the product list are redacted, the redactions leave it unclear whether Lilly signed the same clause, and more than two dozen other MFN contracts stay confidential. That goes to the one lever pharma has sold as its MFN offset. If HHS takes a cut of every foreign price rise, ex-US pricing becomes a lower-quality earnings source for all 26 signatories until the rate is known. It also binds pricing tighter to trade policy, where the UK deal already commits the NHS to 25% higher medicine spending.
