UPFRONT BRIEFING
Rapport's J&J-licensed RAP-219 beat placebo in bipolar mania by 3.1 points, short of the 4-point bar management set itself, and the stock round-tripped a 25%+ premarket pop. Arcus brought 35-month median survival for casdatifan in late-line kidney cancer to its fight with Merck's Welireg, and ICER says about two-thirds of 2022-25 launches were priced past their value.
Money kept moving: Disc turned Monday's Phase 3 into an upsized $450M raise at $75, Lycia set terms for a ~$228M IPO, and OpenAI bought its way into drug-development software. ASN Kidney Week (Denver) and MSToronto open today; ESMO starts in Madrid on Friday.
TAPE ACTION

Healthcare ran to an all-time high and led S&P sectors, XLV +2.4% on large-cap pharma strength with Lilly at a record; development-stage names went further, XBI +3.6%, NBI +3.2% and BBC +4.4% against RUT +1.7%. Rapport's pre-open RAP-219 readout gave back a 25%+ premarket gain after the open.
Money rotated out of tech into value and defensives: Dow +0.95% (~490 points), S&P +0.3%, NDX (0.1%), with GM (7.5%) on below-consensus guidance.
THE BIG 3
1️⃣ — Rapport's RAP-219 clears bipolar mania test by 3.1 points
RAP-219, Rapport's oral TARPγ8-targeted AMPA modulator licensed from J&J, beat placebo by 3.1 points on the Young Mania Rating Scale at week 3 (15.4 vs 12.3) in a 253-patient Phase 2 acute bipolar mania study, p=0.045 per Stifel, with a significant secondary and no severe or serious adverse events; Rapport meets FDA in 1Q27 and plans two registrational trials from 2H27.
Why it matters: The win cleared significance but not Rapport's own bar: CEO Abe Ceesay had called 4 points the clinically meaningful threshold, and Stifel puts the competitive range at 3-4, so RAP-219 sits at the low end on efficacy. The pitch now shifts to tolerability, where adverse events ran at placebo-like rates and Jefferies calls safety clean, worth $1B+ to peak sales if it holds. Adherence decides share in bipolar, and AbbVie's Vraylar is tracking to $4.3B this year on four labels. Shares gave back a 25%+ premarket pop after the open. Open risks are maintenance and relapse data, plus the psychiatric-safety overhang carried by Fycompa, the AMPA comparator.

2️⃣ — Arcus casdatifan posts 35-month survival as it squares up to Welireg
Arcus' HIF-2α inhibitor casdatifan posted 35-month median OS and 13-month median PFS as monotherapy in 121 late-line clear cell RCC patients (52% with 3+ prior lines), 14.5-month median PFS with cabozantinib in second line (n=43) and 69% 12-month PFS with zimberelimab in first line (n=30) at an investor event; the Phase 3 PEAK-20 (with ipilimumab/nivolumab) starts by year-end and RCUS rose about 9.5%.
Why it matters: This is a cross-trial case against Merck's Welireg, the only approved HIF-2α inhibitor, and Arcus built the deck to make it: late-line PFS held flat across one to three prior TKI regimens, where Merck's LITESPARK-013 showed PFS falling as prior lines rose. The first-line doublet's 6.7% primary progression against roughly 30% for Keytruda alone is the number that sells a frontline plan, but n=30 at 11.5 months is thin and every comparator is historical. Gilead let its option lapse in early 2025, so Arcus runs PEAK-1, PEAK-20 and a tivozanib study largely alone, with Taiho holding Japan and most of Asia. The release reported no unexpected safety signals but did not break out anemia or hypoxia, the class's known liabilities.

3️⃣ — ICER says two-thirds of 2022-25 launches priced past value
ICER's launch-price report on 26 drugs it reviewed that won approval in 2022-2025 found about two-thirds launched well above its $100K-$150K per QALY thresholds; the inflation-adjusted median net launch price rose from $190K to $257K, and nearly a quarter of drugs it evaluated in 2025 carried net prices above $500K.
Why it matters: The named drugs carry the read-through. ICER says Insmed's Brinsupri and Merck's Winrevair would each need roughly 90% discounts to hit its thresholds and Carvykti about a third, while Vertex's Journavx already clears and Casgevy at $2.2M needs only about 7%. ICER sets no prices, but payers use its numbers in coverage and step edits, and the report counts four in ten prescriptions for these drugs rejected, worse in Medicare. It lands with more than a dozen MFN-for-tariff-relief deals signed and IRA negotiation running, though BioPharma Dive notes neither has yet dented reported margins. Rare-disease and cell and gene launches, which pushed the 2025 median up, carry the most exposure.
